Out-of-District Student Transfer Form

Parents (legal guardians), who are and will be employees of JCSD, who want to petition JCSD for enrollment for their out-of-district student. Principal and Assistant Superintendent must sign form.

In-District Student Transfer Form

Parents (legal guardians), who are and will be employees of JCSD, who want to petition JCSD for a transfer for their in-district student. Principal and Assistant Superintendent must sign form.

Student Release Form

Parents (legal guardians), who reside in JCSD, who want to petition JCSD to release their student to another school district. Please include in explanation if paying tuition.
