If you are a new vendor that wishes to provide goods or services to Jackson County School District please fill out this form and return it, with the New Vendor Form, to the business office at invoices@jcsd.ms.
Manual Requisition
If you are trying to make a purchase using multiple fund sources, all of which you don't have access within Remote Link, please fill out this requisition and return to the business office with the necessary signatures on it. The business office will then create a PO for you in Marathon.

Sales Tax Exemption - FL
If a vendor is charging the district sales and/or use tax please use this letter to substantiate Jackson County School District's exempt status from sales and use tax.

If a vendor is charging the district sales and/or use tax please use this letter to substantiate Jackson County School District's exempt status from sales and use tax.
Sales Tax Exemption - TX Hotels
Provide completed certificate to hotel to claim exemption from state hotel tax but not local tax. Employees of exempt entities traveling on official business can pay in any manner. This certificate does not need a number to be valid.

Fixed Assets

Asset Number Request Form
When an invoice for the purchase of an asset requiring a JCSD number is submitted for payment, the attached asset number request form should also be scanned and attached to the Req./PO in Remote Link.

JCSD asset numbers will then be assigned after the claim against the PO has been board approved/paid.
Donated Asset Form
When an asset is donated, whether by a teacher using an EEF card or an individual or organization donating property, it must first be board approved. Once this form is completed submit it for board approval. Once board approved please send to the Business Office. A JCSD number will then be assigned.
Lost or Stolen Property Affidavit
This form must be completed and accompany any lost or stolen property that is being discarded from inventory.

Employee Reimbursement

2025 Local Mileage Reimbursement Form
If you are an employee that will be traveling inside of Jackson County, Harrison County, Hancock County, Stone County, Lamar County, Forrest County, and Mobile County then please fill out this form and turn it into the Business Office to be reimbursed for mileage.
2026 Local Mileage Reimbursement Form
If you are an employee that will be traveling inside of Jackson County, Harrison County, Hancock County, Stone County, Lamar County, Forrest County, and Mobile County, after January 1, 2026 then please fill out this form and turn it into the Business Office to be reimbursed for mileage.
CDL Reimbursement Form
If you are an employee that is requesting reimbursement for your CDL fee or CDL medical examination please fill out this form and return to the business office.

Federal Income Tax Withholding Form
If you are an employee that would like to change your federal marital status, exemptions, or additional withholding please fill out this form and return to the business office.
MS State Income Tax Withholding
If you are an employee that would like to change your state marital status, exemptions, or additional withholding please fill out this form and return to the business office.

Time and Leave

Comp Time Addition Form
Please use this form to have comp time added to a noncertified employees leave balance. The form should be signed by the supervisor and sent to the Business Office. The comp time is added in Remote Link.
Overtime Election Form
Overtime Payout Form
Please use this form to have comp time or overtime paid out to a noncertified employee. The form should be signed by the supervisor and sent to the business office. The comp time or overtime is paid out in the next check.
Playoff Per Diem Form
Please use this form to pay playoff per diems. Please email signed forms to Payroll. Please do not duplicate with paper copy.
Summer Job Recommendation Form
Please use this form for recommendations of all Summer Adult/Student Maintenance Jobs and all Summer Adult/Student Floor Crew Jobs. Please place each person under the correct position on the form. The form should then be sent to Human Resources to be add to the next board agenda. Once board approved, the Business Office will set up each job in Marathon.

Activity Fund

Certificate of Exemption from Registration as a Charitable Organization
This certificate exempts the school district from registration as a charitable organization but still allows the school district to accept charitable donations.
Fundraiser and Gifts Form
This form must be completed and have administrative and board approval before entering into any agreement with a vendor, before any advertising and before any solicitation begins. The completed form should be turned into the superintendent's office.
Matching Grant Application Form
If a booster club would like to seek a dollar for dollar match on its donation for capital improvement projects, please complete page 2 and submit the completed form for board approval.
Tax Exemption for Fundraiser Letter
If your school, activity, or club is holding a fundraiser please use this notice to substantiate those goods should be exempt from sales tax.
